Please complete this form before participating in any a4A YEG class, workshop, or program.

For Participants Under 18

I confirm that I am the parent or legal guardian of the participant named above and give permission for them to participate in a4A YEG activities. I have read and agree to the Waiver & Release and Media Consent on behalf of the participant.

Health & Safety

Please let us know if there are any injuries, physical limitations, or other conditions that may affect your ability to safely participate in physical activity.

By participating, I understand that dance and physical activity involve inherent risks, including slips, falls, muscle strains, and other injuries. I agree to participate within my own physical limits and follow the instructions provided by the instructor.

Waiver & Release

By submitting this form, I voluntarily participate in activities offered by a4A YEG and understand that dance and physical activity involve inherent risks, including slips, falls, strains, and other injuries. I agree to follow all studio rules, safety guidelines, and instructor directions and to participate within my own physical abilities.

To the extent permitted by applicable law, I release and hold harmless a4A YEG, its owners, instructors, staff, and representatives from claims arising from my participation, except where caused by negligence or other liability that cannot legally be excluded.

I understand that a4A YEG is not responsible for lost, stolen, or damaged personal belongings.

Media Consent

During a4A YEG classes, workshops, rehearsals, and events, photographs and/or video recordings may be taken for promotional purposes, including social media, the studio website, and other marketing materials.

If you do not consent, a4A YEG will make reasonable efforts to exclude you from promotional photos and videos.
